Tipping norms vary widely: ~15–20% in the US, small rounding in much of Europe, and service is sometimes already included ("servizio incluso") — check the bill before doubling up.

Splitting evenly is the default here and is not always the fair answer. Where one person ordered substantially more, splitting the tip in proportion to each share of the bill keeps the percentage the same for everyone. Splitting the total evenly instead quietly moves money from the light eaters to the heavy ones.

US card terminals increasingly suggest tips calculated on the post-tax total and often start the suggested range at 20%. Neither is a rule — a suggested amount is a default somebody chose. The custom-amount button does exactly the arithmetic on this page, on whichever base you think is right.

Cash and card tips also do not always end up in the same place. In the US a cash tip reaches the server directly and immediately, while a card tip may be pooled across staff, taxed at source and paid out with the next payroll run. Where you want the money to land is a reasonable basis for choosing.
